TAXI DRIVER FUND DISBURSEMENT
IMPORTANT INFORMATION
Hello Drivers,
Those of you in the Tier 1 Driver’s Fund disbursement group were mailed a letter on March 9, 2018. The letter instructed you to bring in a Social Security card along with current valid identification (e.g. Driver’s license or current A-Card) in order to pick up your disbursement check.
*Please be advised* If you cannot find your Social Security card you may bring in another document that shows both your name and Social Security number on it.
Examples of other acceptable documents are:
A previous year’s State or Federal tax return, or other tax document such as a W-2, or form 1099;
A Social Security benefits statement;
A Medicaid card.
